Initiatives (Integrated Kidney Care)
Are you passionate about improving care for patients with chronic conditions? As a Program Manager supporting our Integrated Kidney Care (IKC) model, you will play a key role in advancing how Nurse Practitioners (NPs) deliver high-quality, coordinated care to patients with chronic kidney disease.
In this role, you’ll lead high-impact initiatives that enhance how NPs are supported, trained, and integrated into care teams. You’ll work across clinical, operational, and business partners to design and implement programs that improve patient outcomes, strengthen care delivery, and scale innovative models of care.
This is a highly visible role for someone who thrives in complex environments, enjoys solving challenging problems, and is motivated by making a meaningful difference in healthcare.
Essential Duties and Responsibilities:
Drive Strategic Programs
Lead the planning and execution of key initiatives that improve how Nurse Practitioners deliver care
Build and manage program roadmaps, ensuring alignment with organizational priorities and patient needs
Identify opportunities to enhance care models and bring innovative ideas to life
Solve Complex Challenges
Tackle ambiguous, large-scale problems that impact clinical teams and patient care
Navigate barriers such as onboarding, training, and operational processes
Develop practical, data-informed solutions and drive them through to execution
Collaborate & Influence
Partner with clinical leaders, operations teams, and other cross-functional stakeholders
Build strong relationships and influence decisions to improve care delivery
Lead discussions that align teams and drive progress, even in challenging situations
Track Impact & Improve Performance
Monitor program performance and identify opportunities for improvement
Use data and insights to refine strategies and ensure programs deliver meaningful results
Address social drivers of health and value-based care performance
Lead Through Change
Qualifications
Bachelor’s degree required; Master’s degree (MBA, MHA, or related field) preferred
5+ years of experience in healthcare, consulting, or program management
Proficiency with Microsoft Office, especially PowerPoint and Excel
Proven ability to lead complex, cross-functional initiatives from concept through execution
Strong problem-solving skills with the ability to navigate ambiguity and drive results
Experience working with clinical teams or supporting healthcare delivery models is highly preferred
Excellent communication and stakeholder management skills

The Salary Range for the role is $78,000.00 - $119,000.00 per year.
